Calcific rotator-cuff tendinopathy involves calcium deposits within the rotator-cuff tendons. It can cause episodes of severe pain, particularly during resorption. It is not simply “wear-and-tear calcium,” and deposits can shrink or disappear over time.

- Episodes of severe pain with temporary stiffness
Diagnosis
X-rays usually demonstrate the calcium deposit. Ultrasound can help assess the deposit and rotator cuff. Severe symptoms should still be examined to exclude infection, frozen shoulder or a cuff tear.
Treatment
Initial care focuses on pain control and appropriate exercise/physical therapy to preserve motion and function. Selected patients may be considered for subacromial injection, extracorporeal shockwave therapy or ultrasound-guided needling/lavage. Recent network meta-analyses support several effective options but do not establish one universally superior conservative treatment.
When Is Surgery Considered?
Arthroscopic removal of the deposit and assessment of associated tendon pathology may be considered when substantial pain and disability persist despite appropriate nonsurgical treatment. Most patients do not automatically require surgery.

Is this caused by too much calcium in the diet or blood?
Usually no. The mechanism is not fully understood and should not be reduced to dietary calcium intake.
Does every deposit need to be removed?
No. Many patients improve without surgery, and deposits can undergo spontaneous resorption.
