Symptoms
- Pain and tenderness on the thumb side of the wrist
- Pain with gripping, lifting or twisting the wrist
- Occasional swelling, catching or snapping
- Difficulty moving the thumb comfortably
Diagnosis
Diagnosis is usually clinical. Provocative maneuvers such as the Finkelstein/Eichhoff test can support the diagnosis. X-rays or ultrasound are not routinely required unless the diagnosis is uncertain or another condition needs to be excluded.
Treatment
Temporary modification of painful activities, a thumb-wrist splint and appropriate pain management may be used first. Corticosteroid injection can reduce symptoms in many patients. Surgical release of the tendon compartment may be considered when substantial symptoms persist despite appropriate nonsurgical care.
Postpartum De Quervain
The condition is more common during the postpartum period. It should not be attributed solely to lifting a baby; hormonal and mechanical factors may both contribute. Adjusting lifting technique can reduce aggravation.
